On October 18, the National Health Commission of China issued the “Fourteenth Five-year Plan for National Clinical Specialty Capacity Building” (hereinafter referred to as the “Plan”).It is required to further strengthen the capacity building of clinical specialties through the “Plan”, give full play to the important role of the capacity building of clinical specialties in promoting the high-quality development of public hospitals, and guide all localities to do a good job in building the capacity of clinical specialties during the “14th Five-year Plan”.
The “Plan” proposes that during the” 14th Five-year Plan” period, the central finance will drive local investment and carry out the construction of key clinical specialties at different levels of the country, province, city (county). While directing to support the national medical center and the hospitals managed by ministries to carry out key technological innovation, “Plan” shall implement the “Hundred Million Project” of key clinical specialties to promote the balanced and sustainable development of clinical specialties.
At the national level: Building no less than 750 national key clinical specialty construction projects.
At the national level, in addition to the targeted support projects of national medical centers and the hospitals subordinate to ministries and commissions, at least 150 national key clinical specialty construction projects shall be supported in each province every year, and at least 750 national key clinical specialty construction projects shall be accumulated during the” 14th Five -year Plan” period; the ability of relevant specialties shall reach the international leading level; breakthroughs shall be made in improving the diagnosis and treatment effect of major diseases, reducing the diagnosis and treatment cost, improving the diagnosis and treatment efficiency or breaking the technological monopoly.
At the provincial level: Building no less than 5000 provincial key clinical specialty construction projects
31 provinces support at least 1000 provincial-level key clinical specialty construction projects every year, and the cumulative support during the “14th Five-year Plan” period is no less than 5000.The diagnosis and treatment fields and key technical fields of major diseases affecting people’s health such as malignant tumors, cardiovascular and cerebrovascular diseases and metabolic diseases shall reach the domestic advanced level; advanced diagnosis and treatment concepts and models such as multidisciplinary joint diagnosis and treatment (MDT), integrated traditional Chinese and Western Medicine and rapid rehabilitation shall be widely used; the level of diagnosis and treatment of major diseases and the treatment ability of difficult and critical patients shall be further improved; the ability of prevention, control and disposal of sudden group major diseases shall be further strengthened; the total amount of domestic high-quality medical resources shall be further increased; the distribution of high-quality medical resources shall be more balanced.
At the city (county) level: Building at least 10,000 key clinical specialist capacity-building projects at the prefecture, city and county level.
During the” 14th Five-year Plan “period, 31 provinces shall cumulatively support at least 10000 key clinical specialist capacity-building projects at the prefecture, city and county level. Through the project construction, a grid clinical specialist service system shall be formed, and the diagnosis and treatment ability of common diseases shall be greatly improved to promote the high-quality development of public hospitals.
Focusing on supporting these departments
Specifically, the state will give specific support according to the different conditions of the three-level clinical specialties at the provincial, municipal and county levels, focusing on the construction of weak areas and weak departments.
According to the disease spectrum of Chinese residents, specialties related to major diseases such as malignant tumors, cardio-cerebrovascular diseases, which have a high rate of death and disability and seriously affect people’s health, will be selected for universal construction, so as to ensure the full coverage of related projects in all provinces of the country and further expand high-quality medical resources.
According to the construction basis of clinical key specialties in recent 10 years, combined with the regional characteristics of patients’ cross provincial and remote medical treatment and economic and social development in recent 5 years, the construction will be strengthened for some specialties with high medical demand in different regions, especially in provinces with large population and few national clinical key specialties in the past.
The “Plan” will focus on supporting provinces to strengthen the construction of cardiovascular surgery, obstetrics, orthopedics (including hand surgery and spine surgery), anesthesia, pediatrics, psychiatry and pathology, laboratory, medical imaging and other specialties.
The “Plan” will support relevant specialties to innovate in major diseases affecting people’s health and key technical fields such as regenerative medicine, precision medicine, brain science, artificial intelligence and biomedicine; it is planned to strengthen the construction of compound innovation team through cooperation with universities and research units, strive to solve a number of “neck” technologies, form a number of internationally leading original technologies, and promote the ability of relevant specialties to enter the forefront of the world.
In principle, each province will match one provincial specialty construction project with a population of 1-1.5 million every year.
On the basis of municipal hospitals and county-level hospitals, the construction of basic specialties such as oncology, cardiology, thoracic surgery, general surgery, respiratory, obstetrics, anesthesia, severe, orthopedic surgery, pediatrics, pathology, laboratory, medical imaging, and infectious diseases will be strengthened. By strengthening personnel training and basic investment, promoting minimally invasive surgery such as endoscopic intervention and new diagnosis and treatment models such as MDT and integrated traditional Chinese and western medicine, and strengthening quality and safety management, the “Plan” will continue to improve the ability of clinical specialist services and the level of medical quality and safety, so as to form a specialist service system covering common diseases, frequently-occurring diseases and infectious diseases of residents, so as to ensure the basic needs of the people for medical treatment.
In principle, every year, all localities match one city (county)-level specialist construction project according to a population of 700000, and counties (districts) with a population of less than 700000 will guarantee at least one project during the 14th five-year Plan period.
In this “Plan”, a number of equipment areas have become important focus, especially the cutting-edge technology projects with specialist characteristics and core competitiveness.
The “Plan” proposes to vigorously support the development of minimally invasive techniques, including traditional endoscopic therapy, uterine laparoscopic therapy, interventional therapy, puncture therapy, local minimally invasive treatment, and improvement of surgical methods, and gradually achieve full county coverage of endoscopic and interventional diagnosis and treatment technology.
The “Plan” will actively promote the construction of intelligent medical system, strengthen the exploration and practice of artificial intelligence and sensing technology in the medical industry, promote the new medical service model of “Internet +”, and strive to make positive progress in surgical robots, 3D printing, the application of new medical materials, computer-aided diagnosis and treatment, and telemedicine.
